Payment Options & Forms
PTW is an out of network physical therapy clinic and provides fee-for-service care. Payment in the form of cash, check, Mastercard/Visa is due at the time of service. Receipt of payment and documentation of care plan will be submitted to your insurance company for possible reimbursement. It is recommended that you contact your medical insurance plan for reimbursement criterion. (An Insurance Benefits Worksheet is located in the Forms section and will assist you in this process.)
Fees for Physical Therapy Services:

| Follow Up Appointments | $100.00 |
Wellness Program Fees:
| Individual Perinatal Exercise Training | $50.00/session |
| Group Classes | $75.00/6 sessions |
